Sixteen questions across the eight domains of digital health from my book, Set Up to Care — two per domain — and a score that shows you where the technology in your practice is already doing its job, and where it isn't yet.
Most practices are further along in some domains than others, and it is rarely the domains they expect. The Snapshot asks the two questions per domain that tell you most, scores them, and points you at the chapter that covers each one — so you can start where it will make the biggest difference to your week. It is free, it takes about five minutes, and the answers stay in your browser unless you ask for a copy.
Each of the eight domains opens with a sentence describing what it looks like when that part of your practice is working. It is there to set the scene — you don't score it. Beneath it are two criteria. For each, move the slider to the level that matches your practice today, not where you plan to be. The description under the slider tells you what each level means for that criterion.
If you aren't sure, choose I don't know. That criterion is left unscored and recorded as a gap to come back to — which is a more useful answer than a guess. Answer both criteria in a domain to see its score; answer all sixteen to see your overall score.
The five levels run from 0 to 4:
Every criterion spells out what each level means in its own terms, so you never have to hold these in your head.
A capability that works but isn't documented, owned and checked is Established, not Optimised. The Snapshot doesn't ask you to confirm this criterion by criterion — the full Assessment does — but it is worth knowing what a 4 means before you claim one:
Before you start
How many sites does your practice operate? Optional — but if it is more than one, each criterion gains a follow-up asking whether your answer is true at every site, and you'll get a consistency score alongside your maturity score.
Every domain carries equal weight in the overall score, and both criteria carry equal weight in their domain.

Download a PDF now, or have it emailed to you. Neither is required — your score is already on the screen. Your details are asked for here, after the score, and only if you want a copy sent.
Pressing “Email me this report” sends your answers, your score and the details above to me, and nothing else does. They are used to send you this report and, if you'd like, to follow up about your results — never sold, never shared. Kept for 24 months. The privacy notice has the detail.
